- NOTE 1:
- A corporation shall indicate here the address of its head quarters or main office.
- NOTE 2:
- A corporation shall indicate here its corporate name along with the name of its chief executive officer.
- NOTE 3:
- The type or name of the equipment stated in the technical-standard compliance certificate shall be entered here.
- NOTE 4:
- Pick one of the two alternatives. Where the indication by sentence is selected, write down the exact sentence to be used.
- NOTE 5:
- Write down the number of the technical standard compliance certificate. In an emergency, the appliance data for the Telecommunication Terminal Equipment Evaluation Association can be used. The TTC, however, should immediately be notified of the number of the certificate when the certification is obtained for technical-standard compliance.
- NOTE 6:
- Name of Guidelines for Implementing Interconnection Tests used in an interconnection test must be noted.
- NOTE 7:
- All items relating to TTC standards, as stipulated in the Guidelines for Implementing Interconnection Tests, are to be listed together on a separate sheet and attached to the test results.
- NOTE 8:
- This application form shall be presented in duplicate. A copy of the form is to be returned to the applicant after the TTC has confirmed the contents of the application and made necessary entries in the columns of the form allocated for TTC use.
